pays 100% of your preventive service with an

are eligible to receive two-thirds of the weekly

Voluntary Life Insurance and AD&D –
Optional life insurance is available to
employees, the employee’s spouse, and/or the
employee’s child/ren.

Section 125 Flexible Spending Plan –
Co-pays for office visits and prescriptions,
over-the-counter medications, treatment
programs, and other out-of-pocket expenses can
be reimbursed tax-free. PC On Call provides two
convenient, pre-tax plans to offset expenses
with medical and dependent care options.
Personal Health Days (PHD) – After 90
days of employment, you will begin accruing
supplemental time needed for vacation, sick and
personal business.
